Are you ready to see what Embracing Abilities has to offer your family? To get started with services, please fill out our online intake form. We provide services through the Family Supports Waiver and the Community Integration Habilitation Waiver. We will coordinate with your case manager once the intake form is received. For any immediate questions or concerns, please contact our office at 317-825-TEAM (8326).
