Students a part of LEAF Academy will experience both instruction and place-based learning (PBL) within the classroom. Our team of instructors will work to enhance independent living and vocational skills using research based practices and curriculums.

We do have a non-refundable application fee ($50.00) that must be paid in full before your application will be processed. If you have further questions, please contact info@leaf-indy.org.

Application Fee:

The LEAF Academy application fee is $50.00. After successfully submitting your application, you will receive an invoice from our team. Your application will not be processed until your nonrefundable application fee is paid in full. 

Application Requirements:

All applicants must fit the following requirements in order to be eligible for LEAF Academy.

  • Student must be within the age range of 18-28.
  • Student must live within driving distance of the Embracing Abilities North Clinic (8202 ClearVista Parkway Suite 4A Indianapolis, IN 46256). Transportation is not provided by Embracing Abilities.

Incomplete applications are not accepted; you are not able to save your progress throughout and will need to upload all required documents in one sitting. We recommend you have these documents saved on your laptop where you are able to easily locate and upload.

  • Behavior Support Plan (BSP) – if applicable
  • IEP – if applicable
  • 504 – if applicable
  • Math & Reading Levels

Student Safety & Success:

  • Student must be able to request help independently.
  • Student must be able to independently identify warning signs and labels.
  • Student must use technology appropriately and independently.
  • Student must be able to independently sit safely throughout a ride and display pedestrian-safety awareness.
  • Student must be able to do basic reading and writing tasks.
  • Student must be able to take care of basic needs independently (toileting, meal time, etc).
  • Student is able to do simple additions on a calculator or phone.
  • Student will be able to transfer the skills learned to adult living skills.
  • Student is able to participate appropriately in a medium group setting with males and females.
  • If student uses a wheelchair, we are unable to accommodate non-transfer or assist from slide board as a mode of transfer at this time.

Is this a Full Time Program?

Yes, this program is a full time program. Students will be in classes Monday-Friday.

What ages are appropriate for this program?

We have created this program for ages 18-28 as well as certain minimal reading, writing, math and socialization skills.

Will I need to provide transportation for my students?

Yes, we require students to have reliable and dependable daily transportation provided by family, friends, etc.

Will I need to provide meals and snacks?

Yes, students are required to bring their own meals and snacks daily. We do have a refrigerator and microwave on site. We are also a nut free facility.

Can my student attend for more than 1 year?

Yes, we encourage students to attend the program as long as it's the appropriate placement and they choose to participate.

Can my waiver fund this program?

Unfortunately at this time this program is not designed for waiver funding due to ratio and requirements.

What funding options are available?

Embracing Abilities does not provide scholarships or grants at this time. Please review the opportunities we have found that may be a good fit for you/your student via our Funding and Financial Aid tab.

Will I be able to set up a payment plan?

Yes, you will be able to set up a payment plan.

What documents are required to submit the LEAF Academy application?

The following documents are required to submit your application: IEP or 504 plan, BSP (Behavior Support Plan) if applicable, present math and reading levels.

Is before or after care available?

No, students are to be dropped off no earlier than 15 minutes prior to school start time and to be picked up no later than 15 minutes after class dismissal.

Will students be required to wear a uniform?

Yes, students will be required to wear a LEAF shirt and khaki pants.
